Notice of Privacy Practices

Your Office Visit

After being scheduled for a new patient appointment, you will be seen as soon as possible. Be prepared to bring anything that will help us provide you the best treatment; including medical records, lab tests, and a list of all your current medications. We ask that you bring a copy of your insurance card and a photo ID to every visit. Please arrive 15 minutes before your appointment, unless instructed otherwise.

Appointments

Appointments can be made by calling our office during normal business hours or click here to use our self-scheduling link.

We attempt to stay on schedule to keep you from waiting. Please do not be late for appointments. We appreciate your patience for the occasional emergency which may delay your visit. If you cannot keep an appointment, please let us know immediately so others can be seen and keep our wait time to a minimum.  A $25.00 fee will be assessed for a missed appointment.

Phone Calls and Prescription Refills

Phone calls will be returned by the nurse as soon as possible.

We request that first you verify with your pharmacist that there are no more refills available. Have your pharmacy phone number available when you call.  Because we need your chart to refill most medications, routine refills will not be phoned in after hours or on weekends. Prescriptions are renewed during normal office hours. Please give 24 hours notice for all prescription requests. In many instances, we will require that a patient be seen in our office prior to medication renewal.

After Hours & Emergency Care

If you are experiencing a life-threatening emergency, dial 911 or go to the nearest emergency room.  For established patients, OB and GYN emergency care is available after hours.  On-call nurses with physician back-up are available 24 hours a day 7 days a week.

Please use our after hours nurse for OB/GYN related emergencies only.  If an after hours call is deemed non-emergent, a fee of $25 will be billed to you.

Financial

All co-payments and deductibles are payable at the time of service. We accept cash, checks, and most major credit cards and debit cards. Please contact our billing office at (205) 271-5698 should you have any financial questions or concerns.
